IN HOME NURSING SUPPORT
Our highly qualified and friendly nursing team can provide your client with a broad range of nursing services. Click on the arrows next to each nursing service heading to view more information.
We play an important role in ensuring that people who live at home receive suitable information and/or assistance so your clients can take their medicines as prescribed. Our home nursing team liaise between family GPs and local pharmacists, to deliver safe and effective medicines administration.
This essential service is particularly important when a person is struggling with memory loss or has Dementia, and is unable to take responsibility for their own medicines, or their carer needs help in managing and administering their loved one’s medicines.
All consumers will be assessed on their ability to manage their medications at the first visit and as care needs change, and in accordance with relevant program guidelines to ensure care needs are met.
Wound prevention and management is available to treat chronic and complex wounds. We offer quality wound care practice covering:
- Assessing and management of wounds and skin tears
- Pressure assessment & monitoring
- Care of wound
- Ostomy care
- Apply creams, lotions & ointments
- Wound care education and dressings
- Graduated Compression Therapy (GCT)
We aim to explain and manage behaviours that improve skin integrity, optimise healing and prevent possible recurrences.
Empower Nurses are trained in Graduated Compression Therapy (GCT) which is a carefully designed approach for the prevention and management of venous return (the rate of blood flow back to the heart) and to reduce oedema (build-up of fluid in the body). Read more about GCT in this blog
For client that are susceptible to developing pressure ulcers, our experienced nurses can monitor and manage and maintain healthy skin including position regimes for consumers whose activity is limited to bed, chairs or wheelchairs.
We can help your clients actively manage their diabetes, so they can continue to live a full lifestyle at home. We offer a broad range of care services to help meet the challenges of managing diabetes. Our diabetes management services include;
- Blood sugar monitoring via Nursing
- Medication management via Nursing
- Nutrition assistance via Dietetics
- Diabetic foot care via Podiatry
Empowers Home Care Nursing services can support clients at home with the management of continence problems. We offer a service that aims to preserve dignity, reduce stress and regain confidence so your client can participate in daily activities and get back to enjoying life.
Our services cover:
- Basic Level 2 RN continence assessment
- Continence related skin integrity concerns
Empower Nurses will conduct a comprehensive interview style assessment in the client’s home and advise on suitable individualised treatment options to assist keeping clients at home longer. Management will depend on the type of incontinence, its cause, the severity of symptoms, as well as consumer factors such as cognition, functional status, and medical history.
Is your client missing out on funding? Did you know that we can complete Cognitive assessments to determine their eligibility for the Dementia and Cognition Supplement?
Your client may be eligible for an extra 10% on top of the Home Care Package funding to keep them at home for longer.
Our Nursing Team complete the assessment using validated tools (PAS or RUDAS) and provide you with the assessment outcome on the same day.
This assessment is completed within a 1 hour visit.
Our post-hospital nursing care is designed to support consumers recovering from a medical procedure or illness and avoid hospital readmission.
We can help ensure a faster recovery by:
- Medication administration
- Wound management to prevent infection
Following discharge from a hospital stay, we will consult with your client’s doctor to personalise the nursing service they need to restore their health and independence at home.
Our caring and respectful nursing team are experienced at delivering care support services for terminally ill patients in their own homes or the home of relatives, or care homes for the frail or aged – patients not in a hospital or in-patient palliative care unit.
We aim to provide comprehensive support for the person, their family and other carers, and support which embraces physical, psychological and spiritual needs.
Multidisciplinary care is available to palliative care patients to assist with control of pain and symptoms, hospital or hospice admission when required, equipment and social support.
Empower’s nursing team support all ages of people managing catheters. Whether it’s younger people who manage their own care, to adults with complex needs, older people who manage their own care, and frail older people with complex needs, our nurses are skilled and knowledgeable in this area with the prevention and management of common issues arising from catheterisation.
With the help of our nurses, you client can receive the stoma care needed in the privacy and comfort of their home, without needing to visit a hospital.
Our nurses are experienced in managing stomas and will educate your client on diet, exercise, and how to avoid complications through techniques, medications, aids and appliances.
Our nurses will help your client:
- understand how to care for their stoma
- solve any issues that arise
- learn what other resources are available to them in the community.
Our Nurses provide support to suit each person’s unique circumstance including assisting with personal hygiene activities such as
